Initial Evaluation
The initial evaluation will be scheduled for 60 minutes. During these initial appointments, we will discuss your current concerns, review your history, identify treatment goals, and collaborate on an initial treatment plan. We will work together to set treatment goals and create a plan to help you reach those goals. This plan may include medications, behavioral strategies, therapy and/or referral to other providers when necessary. Please give our office a call and schedule your next appointment for a psychiatric evaluation.
FOLLOW-UP EVALUATIONS
At each appointment, we will evaluate the effectiveness of the medication along with your current symptoms, goals and coping strategies. We will discuss any concerns you may have.
HOURS
Hours will vary. Saturday and evening appointments, as well as Concierge and Telepsychiatry are available.
Insurance and Fees
I am currently accepting new clients with the following insurance carriers:

- Blue Cross / Blue Shield
OUT-OF-POCKET FEE SCHEDULE
- 60 minutes Scheduled appointments (50 min with provider): $300
- 30 minute scheduled appointments (20 min with provider): $150
PAYMENT
- Payment is due for each visit at the end of your appointment. We accept Master Card, Visa, American Express, and Discover debit and credit cards. Checks should be made payable to “New Horizon Mental Health and Wellness”.
Sliding Fee Discount Program
At New Horizon Mental Health & Wellness LLC, we believe that everyone deserves access to quality mental health care, regardless of their financial situation.
We offer a Sliding Fee Discount Program (SFDP) that reduces the cost of our services for eligible patients. Eligibility is based on your household income and family size, in accordance with the current Federal Poverty Guidelines published by the U.S. Department of Health and Human Services.
No one will be denied services due to inability to pay.
Who is Eligible?
Any patient may apply for our Sliding Fee Discount Program. You may be eligible for reduced fees if your household income is at or below 200% of the Federal Poverty Level. Both uninsured and insured patients may qualify — the discount can apply to your out-of-pocket costs, including copays, coinsurance, and deductibles.
How Does It Work?
Our fee discounts are based on a sliding scale with the following tiers:

| At or below 100% FPG | Nominal fee of $25.00 per visit |
| 101% – 125% FPG | 75% discount off standard fees |
| 126% – 150% FPG | 50% discount off standard fees |
| 151% – 175% FPG | 25% discount off standard fees |
| 176% – 200% FPG | 10% discount off standard fees |
| Above 200% FPG | Standard fees apply |
How to Apply
- Complete a Sliding Fee Discount Program Application. You can request one from our front desk staff, or download it from this page.
- Provide documentation of your household income (such as a recent tax return, pay stubs, or benefit statements). If you cannot provide documentation, you may self-attest to your income.
- Our staff will review your application and notify you of your discount tier, typically within five (5) business days.
Your eligibility will be reassessed at least once per year, or if your income or family size changes.
What You Need to Know
- No proof of citizenship, residency, or immigration status is required.
- All financial information is kept strictly confidential.
- Your discount status will never affect the quality or availability of your care.
- You have the right to apply for the program at any time.
- You have the right to appeal any eligibility determination.
